If you look at the printed receipt, the REAL tip he gave was 18% - $4.49.

Whoever did this just crossed that out and wrote the other stuff in in pen so he could make a crappy joke on the internet. The payment was credit or debit and the printed receipt shows what the server actually got.

That's an automatic gratuity. Doesn't mean the image isn't a fake, but, really, you don't know the difference between automatic and manual gratuities?

You're half right though, someone here doesn't know how it works.

Yes, I know sometimes they're blank. On larger checks or with larger parties, sometimes the gratuity is filled in automatically. That does not, however, prevent you from being able to override the tip with a larger one.

Personally, I always pay whatever the automatic tip is and then leave like 20 percent on the table in cash.

All the receipts in Canada are the blank ones. Tips are never automatic here, unless you're in a really expensive place and the bill is high and even then, it's listed as a line item in the bill. You would be presented with either blank-total receipts to sign (so you can add additional tip), or just a fixed total with a signature line (like a non-restaurant bill).

As I understand it, a write-in number ONLY matters on blanks. If you do the transaction entirely using the pin pad, then it's just processed electronically, the fixed totals are printed on the line, and you can't write in an alternate total over top. That's intentional, to prevent waitstaff fraud.

Because there's a line for extra gratuity and to fill in the total. As shown in those images I just linked. I have never seen a completely filled in check except that photoshopped one, for exactly the reason mongrel gave.
