If you're defending E from a place where a spy can get you, you're pretty much doing it wrong or doing super right because the enemy can't get out of their spawn.

The only time that kind of shit didn't happen was the short time in which the Pyro's damage drop off was taken away, and thus the ignition/damage area was a cone (or maybe a streched out pyramid on its side) in front of him.  Then they added in the drop off again, and the flamethrower reverted to being melee weapon with a slightly longer reach than most.
